To make a Singleness Affidavit for marriage (only for applicant holding residence visa not for visit visa holders), these are the requirements.

Personal presence of the person who is making the Singleness Affidavit (Typing will be done at the SGIVS Center)
Passport in original and copy of the applicant (consular signature page / first / last and valid residence visa page clear copy).
Fiancé/Fiancée’s passport copy
In case of a divorcee applying for a singleness affidavit, divorce papers issued under mutual consent by the court from the authority where the marriage was registered are required
