Sworn Affidavit Attestation

• Physical presence of the applicant executing the Sworn Affidavit is required.
• Passport in original and copy of the applicant (consular signature page / first / last and valid visa page clear copy). {Photocopy facility is available at the center}
• Sworn Affidavit in the prescribed format along with a photocopy (the form will be typed at the center). (This document has to be signed in the presence of the Consular Officer).
• Personal presence of the applicants / applicant along with the passport in original with photocopy (2sets) (first / last and valid visa page).
• The charges will be applicable to all applicants as signed
• In case you are applying for a Sworn Affidavit for an OCI card, a copy of the passport must be duly attested by the Ministry of Foreign Affairs (MOFA)
