Life Certificate Attestation

Personal presence at the SG IVS Global, of the pensioner with the following documents:

Life Certificate (Typing services available in the center)
Name as in Passport – Given Name followed by Surname
Father’s Name as per the passport on the last page
Permanent Address as per the passport on the last page
Passport in original with photocopy (consular signature page / first / last and valid visa page clear copy).
Copy of Pension Order
Duly typed Life Certificate Form (This can be done in our office ) ( Format is given below)

The attestation fee is waived for pension payment holders only. SGIVS service fee only applicable.

Copy of Pension Payment Order is required to waive the attestation fee for pension payment holders

Special Instructions to fill the form:

A legible copy of Pension Payment Order having details of verifiable information like father’s name, Date of Birth etc.

Other add-on services will be charged.

Courier to India	:	AED 94.50
