Personal presence at the SG IVS Global, of the pensioner with the following documents:

Passport in original with photocopy (consular signature page / first / last and valid visa page clear copy).
Copy of Pension Order
Duly typed Life Certificate Form (This can be done in our office ) ( Format is given below)

Attestation fee is waived off for Ex-servicemen only. However, a service fee of AED 4.50/- is chargeable.
(Discharge Book/ Ex-Servicemen Identity Card are required to waive attestation fee for Ex-Servicemen)
Additional copy of the Life Certificate to be attested will bear the total applicable fee of AED 50.50/-

Special Instructions to fill the form:

Life Certificate to be typed. (typing service is available )
Name as in Passport – Given Name followed by Surname
Father’s Name as in passport on the last page
Permanent Address as in passport on the last page
A legible copy of Pension Payment Order having details of verifiable information like father’s name, Date of Birth etc.
