To make a Sworn Affidavit these are the requirements:

Personal presence of the person who is making the Sworn Affidavit.
Passport in original and copy of the applicant (consular signature page / first / last and valid visa page clear copy).
Sworn Affidavit in the prescribed format along with a photocopy (the form will be typed at IVS office). (This document has to be signed in the presence of the Consular Officer).
Personal presence of the applicants / applicant along with the passport in original with photocopy (2sets) (first / last and valid visa page).
